It is possible to see all three visions in one encounter. If Phase 2 can last long enough, you may go through all three visions. There is no real strategy to this achievement as the portals are completely random. However, if you do not get through the vision and into the brain room within 60 seconds, you will be driven insane and become mind controlled.
