Objective: Opened the gates of Ahn'Qiraj.
Criteria:

You would have had to be the first person on the realm to complete the Bang a Gong! quest during the Gates of AQ event. Unless you are on a brand new server, it is impossible to attain!

UPDATE

Technically anyone can still obtain the FoS within 10 hours after the first person completed the quest. But as the description suggests, you cannot get it anymore because;
1: All servers already have this event completed.
2: The upcoming servers will already have the gates opened.
3: The quest-line has been removed from the game as of 4.0.
